Our Nursing Home Transition and Diversion (NHTD) Program is designed to help those who are either at risk of entering a nursing facility or who wish to transition from a nursing facility back to their homes. What is the NHTD Program?

Our NHTD (Nursing Home Transition and Diversion) waiver program services offer a range of essential services to help individuals live independently in their own homes and communities, promoting greater freedom and self-determination. With our expert in-home health aides and caregivers, you can trust that you or your loved one will receive personalized, high-quality care for traumatic brain injury, disability, dementia, and other caregiver support services for the elderly. 

Who is Eligible?

Eligibility for the NHTD program includes:

  • Individuals of any age who require the level of care provided in a nursing home available at home through an in-home health agency.

  • Those who are capable of living safely in the community with assistance from the NHTD waiver services.
