Nursing Home Transition and Diversion (NHTD) Waiver Program
At RESPONSIBLE HOMECARE, we are dedicated to supporting individuals in their desire to live independently. Our Nursing Home Transition and Diversion (NHTD) Program is designed to help those who are either at risk of entering a nursing facility or who wish to transition from a nursing facility back to their homes. Our goal is to provide the support and services necessary to enable a safe, comfortable, and dignified living environment within the community.

What is the NHTD Program?
Our NHTD (Nursing Home Transition and Diversion) waiver program services offer a range of essential services to help individuals live independently in their own homes and communities, promoting greater freedom and self-determination. With our expert in-home health aides and caregivers, you can trust that you or your loved one will receive personalized, high-quality care for traumatic brain injury, disability, dementia, and other caregiver support services for the elderly.
